Home Care Services Buffalo MNFamily Support-Relief Care Nursing Aide:

Provides families with an occasional break from caregiving responsibilities. Support can be provided for any level of care, from a few hours to 24 hours a day.

Medication Management and Set Up:

(up to 1.5 hrs.)

  • A licensed nurse will set up medication for easy administration and order refills as needed.
  • The nurse will provide medication education to the client and family as prescribed by a physician.
  • Communicate with the client, family, doctor, pharmacist, and other health care professionals.
  • INR testing for blood anti-coagulation therapy.

 

Wake-up and Tuck-in Service:

Morning and Evening Care Package:

(Total 3 hrs. each day)

Personal care from a nursing aide for 3 hours total to be divided in the morning and the evening.

  • Medication reminders
  • Bathing & dressing
  • Meal preparation
  • Household laundry
  • Light housekeeping
  • General assistance related to care
  • Pet care

Independence at Home:

(4 visits per week; a total of 7 hrs. of care per week)

Once a week, a licensed nurse will perform a nursing assessment and complete medication management. Up to a 2-hour nursing aide visit three times per week to help with personal and companion care.

  • General assistance related to care

Aging Life Care-Health Care Coordination:

  • Attend physician appointments and provide assistance
  • Observe and record results in a health journal
  • Organize prescription delivery where available
  • Arrange for home modifications and medical supplies
  • Coordinate community resources and services
  • Communicate with other health care professionals and family
  • Emotional support for the family

Home Safety Evaluation:

A Physical or Occupational Therapist will evaluate the home environment and provide recommendations for clients to optimize their home relative to individual abilities and help promote full participation in daily life activities.

Evaluation includes:

  • Identifying fall risks
  • Identifying the need for adaptive equipment, such as grab bars or handheld showers
  • Assessing mobility with assistive devices, such as walkers, canes, or wheelchairs
  • Evaluating kitchen safety
  • Assessing stair safety
  • Discussing personal emergency response system

Recommended home modifications may include:

  • Ramps
  • Lighting
  • Railings
  • Walk-in tubs
  • Stair treads
  • Moving furniture for ease of mobility
  • Personalized Packages

Personalized Packages

Care requests can be customized into a care package personalized for the client. PRHC does not require a minimum number of care hours. Each personalized care plan will be carefully considered and together you and your nurse will create a plan that is right for you.
